Intervention Coordinator (IC) (40 hours, Full-time) Non-exempt

The Intervention Coordinator (IC) will supervise the intake process and act as the first point of contact for youth, their families, and external partners seeking BUILD services. In addition will provide support to the Manager of Intervention Services and the Intervention Team to ensure they are servicing youth through the process of assessing, planning, implementing, coordinating, monitoring and evaluating options and services required to meet the client's service plan needs and goals around mental, physical and general well-being. Through advocacy, communication and resource navigation, the Intervention Team will promote equality and cost-effective interventions and outcomes helping find services needed and monitor progress through the completion of service plans. The (IC) reports directly to the Manager of Intervention Service (MIS) .
Responsibilities are but not limited to :
- Provide and / or coordinate intake services and process with relevant screening to identify service needs
- Recommend care options and coordinate a warm transfer with Intervention Navigator
- Track intake information in agency referral tool and collect data to include in regular reporting schedule
- Maintain HIPAA protocols and client confidentiality throughout the intake process
- Meets with Intervention Navigators weekly to review caseloads and documentation.
- Reports to Manager of Intervention Services regularly in preparation for quarterly case review
- Contributes to participant related data systems for record keeping and grant reporting purposes
- Compiles reports and ensure compliance of team members
- Ensure the timely flow of case management activities to meet program service plan goals
- Utilizes BUILD Universal Referral form to document and connect families to other BUILD services as needed
- Operates professionally in the field on a flexible schedule, which may include late evenings and weekends
- Ensure Navigators document all forms required, including case notes, youth service plans, referral forms, information releases related to health or disabling conditions, health information, and any other information necessary in program participants file utilizing all procedures related to maintaining confidentiality at all times
- Reviews and provides feedback to Manager of Intervention Services regarding data collection
- In partnership with Crisis Response Specialist, Community Wellness Navigator and Mental Health Therapist, helps to identify positive adult / youth influences to create a stronger support network for the program participant
- Regularly monitors client activity related to achieving personal and programmatic goals, and provides revisions to service plans as needed
- Ensure navigators have a regular schedule of home visits, contacts, and follow-ups to build relationships with youth and ensure completion of service plans
- Represents BUILD to broaden community awareness and support, interpreting the agency's programs and services at program-related community, coalition and committee meetings as needed
- Attends departmental and agency-wide staff meetings, as scheduled and agency-wide events, as required
- Maintains knowledge current in the field in way of participation of coursework, seminars and / or workshops
- Contribute to data reporting request collaboration with program manager and coordinator
- Perform other duties as assigned
