The Lewelling Shelter and Drop-In Center, set to open in March 2025, will serve chronically homeless individuals. The center comprises 29 interim - shelter rooms and offers drop-in services (showers, laundry, meals, meetings and individual meetings) five days weekly.
POSITION SUMMARY: The Interim Housing Case Manager provides case management services for chronically homeless individuals at emergency shelter, including assistance with obtaining housing, developing life skills/income, and achieving greater self-determination. The Interim Housing Case Manager is supervised by the Program Manager and works closely with the Site Coordinator, and as part of a collaborative service team.
ESSENTIAL FUNCTIONS
- Conduct program participant intake, including assessments and safety planning
- Conduct orientation sessions for new guests/program participants
- Develop, complete and maintain client files, case notes and incident reports
- Assess individuals and families housing barriers, to determine housing and service needs
- Develop comprehensive service plan for each program participant; monitor progress and service delivery
- Provide weekly and ongoing case management services
- Conduct room checks/wellness visits
- Building relationships with community providers to be able to refer clients to resources/services
- Assess and refer program participants to mainstream resources and support programs including life skills, employment, mental health, transportation, and education programs; advocate to help clients successfully access services
- Conduct at least two education/support group meetings weekly
- On a rotating basis, provide 24-hour on-call response for shelter site
- Participate in weekly case conference meetings to review and discuss client and service delivery issues
- Attend outside meetings representing the agency as needed
- Maintain confidentiality of all guests and program participants
- Maintain accurate client records, collect statistics, and submit monthly reports.
- Provide shift coverage as needed.
- Assist Site Coordinator with upkeep on shelter operations
- Attend and participate in shelter and agency staff meetings as scheduled.
- Co-facilitate weekly house and monthly staff meetings with Site Supervisor
- Coordinate with other sites and programs within the agency
- Other duties as needed
QUALIFICATIONS
- Minimum of two years’ case management and/or direct service delivery, preferably working with issues of homelessness and/or domestic violence, substance abuse, mental health, and child abuse prevention
- Understanding of and sensitivity to the needs of those experiencing homelessness, mental health issues, and trauma
- Demonstrable history of handling service provision, documentation, rule enforcement, and crisis intervention with multiple interruptions and multi-task projects
- Knowledge of Alameda County resources for homeless and underserved populations
- Excellent oral and written skills
- Certification required as Certified Domestic Violence Counselor (training offered soon after hire if needed)
- Ability to complete statistical reports in a timely manner.
- Available to work occasional weeknights and/or weekends as needed
- Valid California Driver’s License and access to personal automobile
