SUMMARY
It is the responsible of the Wraparound Facilitator to provide services and to oversee all aspects of the clients needs who are assigned to them.
EDUCATION
- Must be at least 21 years old and have a Bachelors Degree in a Human Services related field.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
- Must possess a Bachelors Degree.
- Must maintain an appropriate amount of continuing education per policy.
- Must have a valid state issued drivers license and submit to a drug test and an intensive background investigation.
DUTIES AND RESPONSIBILITIES
The duties and responsibilities of the Wraparound Facilitator of Burlington United Methodist Family Services shall encompass at least the following:
- Provide intensive case management services to youth, aged 12-17 (up to the youths 17th birthday) with a diagnosis of a severe emotional or behavioral disturbance that impedes his or her daily functioning (DSM-V Axis I) who is who is currently in out-of-state residential placement who cannot return home without extra support, linkage, and services provided by wraparound; who is currently in in-state residential placement who cannot be reunified without extra support, linkage, and services provided by wraparound; who is at risk of out-of-state residential placement and utilization of wraparound can safely prevent the placement, or who at risk of in-state level one, two or three or Psychiatric residential placement and utilization of wraparound can safely prevent placement.
- Provide oversight of services for the client to ensure his/her safety and well-being.
- Provide coordination of services that will include but not be limited to assessment and evaluation, individualized wraparound service planning, intensive case management, outpatient therapy to include individual, family and group, day treatment, behavior management, intensive home-based mental health services, school-based behavioral health services, substance abuse, intensive outpatient services, crisis services, mobile crisis response, youth coaching, peer support, respite services, therapeutic mentoring and therapeutic foster care.
- Initiate services to ensure the safety of the youth and family.
- Shift families from professional to natural supports to help families build long-term connections and supports in their communities.
- Provide concurrent community work while youth is in the residential care for a smooth transition home.
- Reduce the occurrence and negative impact of traumatic events in the childs life.
- Ensure the child/youth receives crisis stabilization without the need for youth to enter reenter residential care.
- Reduce the length of stay in congregate care.
- Develop service plans that are individualized and tailored for each family that are strength based that unifies residential and community treatment, creating a seamless transition as youth and families move through the system;
- Encourage innovation, creativity and appropriate goal setting.
- Ensure adherence of performance measures for families utilizing Safe at Home Wrap-around.
- Ensure assessments are completed on every family member
- Maintain a caseload of not more than 10 youth and their families.
- Meet with the clients and their family for a family team meeting to include not less than one face-to-face per month. (Meetings could be daily, weekly or monthly according to the needs of the youth and family)
- Provide a continuity of care which is best for each youth and his/her family.
- Ensure wraparound services are coordinated, created, and individualized and supports to fit the unique needs of the child and family while building upon their strengths.
- Assist with problem-solving, manage disagreements that may occur during the wraparound process, assessment, and advocating on their behalf.
- Assist in enhancing cross system collaboration and assure early linkage/intervention
- Work in conjunction with other service providers.
- Act as a liaison between service providers.
- Complete assigned tasks in a timely and competent fashion.
- Provide proper documentation and timely completion of all paper work associated with the requirements of the position.
- Communicate all pertinent information to your supervisor.
- Establish a positive working relationship with all members of the team. Work cooperatively with all staff for the good of the agency/youth and family, and project a positive public image.
- Attend all required meetings and necessary case reviews.
- Provide monthly and quarterly reports.
- Be prepared to discuss issues and to give a written response when needed.
- Be responsible for ensuring all clients have a discharged plan.
- Promote team building and teamwork.
- This position is defined as one that will require you to drive a motor vehicle as part of your daily work.
- Perform other duties that fall within the parameters of this position.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee will be required to lift, stand, sit, and/or walk for extended periods of time. Repetitive use of hands, wrists, and elbows will be required. The employee will be required to possess the strength and ability to demonstrate techniques taught in various required training.
This position will work in an office environment and work in extreme weather conditions and may be working in potentially hazardous situations due to working in the clients home and community.
