KVC West Virginia is looking to hire is dedicated to providing personalized, family-centered services for children and families with complex needs. Our mission is to offer support in navigating systems and connecting families with necessary resources to achieve long-term stability and success. This position will serve Jackson County and surrounding areas.
Position Overview:
We are seeking a compassionate and dedicated Wraparound Facilitator to work with families and children to help them navigate the wraparound process. This position requires a strong commitment to building relationships, creating individualized plans, and coordinating community-based services. The Wraparound Facilitator will be responsible for guiding families through the wraparound process, ensuring they receive the appropriate services and support to improve their overall well-being.
Key Responsibilities:
- Facilitate wraparound meetings and planning sessions with families, children, and service providers.
- Develop individualized, strengths-based service plans for families.
- Coordinate and connect families with community resources, mental health services, educational support, and other necessary services.
- Monitor the progress of the wraparound plan, adjusting as needed based on family needs and feedback.
- Collaborate with a multi-disciplinary team of professionals, including case managers, therapists, educators, and service providers.
- Provide support and advocacy to families to ensure their voice is heard throughout the process.
- Help families develop coping strategies and problem-solving skills.
- Maintain detailed case notes and documentation in compliance with company policies and regulatory requirements.
- Evaluate the effectiveness of wraparound services and make recommendations for improvements.
- Ensure a culturally competent and trauma-informed approach in all interactions.
Qualifications:
- Bachelor's degree in social work, psychology, human services, or related field (required).
- Previous experience working with children and families in a social service, mental health, or community-based setting (preferred).
- Knowledge of the wraparound model and systems of care.
- Strong communication, organizational, and interpersonal skills.
- Ability to work independently as well as part of a collaborative team.
- Demonstrated cultural competency and sensitivity.
- Ability to manage multiple tasks and deadlines effectively.
- Experience with case management or service coordination is a plus.
- Ability to maintain confidentiality and adhere to ethical standards.
