What are the responsibilities and job description for the Intake and Referral Specialist - 2273001 position at Butler County Board of DD?
- Oversees all referrals to the SSA department from Eligibility and coordinates all incoming county to county transfers; completes Pre-Admission Counseling as needed.
- Assures individuals are receiving minimal supports and are monitored for increased service needs. Determine whether criteria are met to be placed on an age based SSA caseload.
- Assists families with providing explanations and answering questions regarding paperwork required from other agency systems.
- Assures the completion of written reports, case notes/chronological entries, correspondence and other written documentation as required.
- Acts as a resource for individuals and families by providing information and making referrals to other agencies or service providers; may include attending meetings with families and assisting with paperwork/applications.
- Maintains awareness of social service agencies and community resources. May communicate or coordinate service supports with other agency and school district representatives. Maintains an updated Resource Library of community resources to meet the needs of families/individuals.
- Maintains accurate Individual Demographic System (IDS) information for individuals/families receiving only Family Support Services Program (FSSP).
- Knowledgeable with the eligibility and referral process and other systems (social security, Medicaid, Medicare).
- Maintains monthly productivity requirements.
- Works cooperatively with individuals, families/guardians (as applicable), providers, and other stakeholders to monitor services.
- Works with other Board departments and divisions to solicit feedback, communicate findings, and address unmet needs.
- May be asked to provide training or support to co-workers, individuals, families, and/or providers when they have attended a training or event and the information received would be beneficial to or needed by others.
Education
Required- Bachelors or better in Human Services or related field
