JOB TITLE: Referral Specialist
DEPARTMENT: Admin
REPORTS TO: Call Center Supervisor
STATUS: Non-Exempt
SUMMARY:
This role is key in connecting patients to specialty providers in order to provide comprehensive care. The Referral Specialist is responsible for guiding the patient through the referral process to ensure the patient attends the specialist appointment and the Primary Health Solutions (PHS) provider receives the consult note for their review.

- Track and log referral follow ups in i2i
- Contact patients to follow up on their referral status
- Connect patients to specialty providers taking into consideration geographic location, insurances accepted, appointment availability and patient preferences
- Close referral loops after successful receipt of specialty consult note or after multiple follow up attempts with the patient
- Collect and analyze specialist information including office hours, insurances accepted, appointment availability and timeliness of response

QUALIFICATIONS:

EDUCATION AND/OR EXPERIENCE:
Minimum of a high school diploma or equivalent required. Graduation from an accredited Medical Assistants program preferred. 2-3 years of experience in a clinical setting.
LANGUAGE SKILLS:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write clear and concise routine reports and correspondence.
REASONING ABILITY:
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
COMPUTER SKILLS:
To perform this job successfully, an individual should have the ability to gain knowledge of current practice management system, electronic medical record, Microsoft Word, Internet, and Intranet. Must also be proficient in Microsoft Word, PowerPoint, and Excel.
CERTIFICATES, LICENSES, REGISTRATIONS:
Certified or Registered Medical Assistant or valid Ohio nursing license if applicable.
OTHER SKILLS, KNOWLEDGE AND ABILITIES:
Ability to speak Spanish desirable. Excellent communications skills, both written and verbal. Ability to work effectively and independently with all levels of clinical and administrative staff within the health centers and with community providers. Knowledge of examination, diagnostic and treatment room procedures. Knowledge of medical terminology.
