GENERAL DESCRIPTION:
The Community Wraparound Facilitator should believe that all youth & families have strengths and deserve access to high-quality services and supports. We are seeking a creative, empathetic, resourceful and flexible individual who is committed to doing what it takes to support & empower youth and their caretakers through challenges. Facilitator will work collaboratively with families, other professionals, and community partners to develop individualized, family-determined, strength-based plans of care. Responsibilities include assisting families in navigating health and human services systems, developing a plan of care, overseeing the implementation of the plan, identifying providers of services or family-based resources, facilitating team meetings, and establishing networks of support and services within their community.
QUALIFICATIONS:
Minimum of Associate’s degree in Human Services or related field. Bachelor's degree in Human Services Field preferred. Two or more years prior social service experience working with children and families with complex behavioral or developmental needs.
Experience in mediation and/or group facilitation preferred.
Excellent communication, organizational, problem solving & team-building skills.
Ability to operate with a non-judgmental attitude and respond with empathy.
Familiarity with or willingness to learn about the child-serving systems and resources for youth in Butler County.
Good organizational skills and ability to work independently without direct supervision.
Ability to handle stressful situations.
Understanding of ethics and boundaries in serving youth and families.
Computer literacy.
History of good job attendance.
Valid driver’s license and access to reliable transportation.
Must pass a criminal background check, and be free from communicable disease.
TERMS OF EMPLOYMENT:
Salary and work year as established by the Governing Board. The employee shall remain free of any alcohol or non - prescribed controlled substance use in the workplace throughout his/her employment in the District.
EVALUATION:
Performance of this position will be evaluated in accordance with provisions of the Board’s policy on Evaluation of Support Personnel.
