BUTLER COUNTY HEALTH CENTER
JOB TITLE: HIV Case Manager

IMMEDIATE SUPERVISOR: Administrator
Summary: Participates in providing services to HIV infected persons. Work involves travel to promote awareness to HIV infected and affected in the region, develops service plans, and to provide evidence-based intervention programs, community outreach, community planning and counseling/testing to those at risk of exposure. In addition, the employee assists with community awareness and education.
ESSENTIAL FUNCTIONS
I. Education Related Activities 90% of Time
- Maintains an understanding of the HIV related health and psycho-social needs of affected individuals in the community and its education principles.
- Provides case management services to affected individuals under the framework of the Missouri DHSS ensuring professionalism.
- Conducts comprehensive assessments with clients and /or family/support groups ensuring biopsychosocial cultural sensitivity.
- Arranges and coordinates multiple services for clients and families.
- Provides ongoing monitoring of the activities implemented to carry out plans of care ensuring thoroughness and timeliness.
- Conducts evaluations to determine effectiveness of plan of care ensuring timeliness.
- Provides education and supportive counseling about the medical and psycho-social aspects of HIV/AIDS ensuring professionalism.
- Maintains HIPAA safeguards, by using and disclosing health information only as authorized.
II. Community Outreach Activities 10% of Time
- Collaborates with other health specialists and civic groups to determine health needs, develops desirable health goals, and determines availability of professional Health services.
- Develops and maintains cooperation between public civic, professional, and voluntary agencies. Ensures professionalism.
- Prepares and disseminates educational and informational materials through health fairs, slideshows, website updates, etc.
- Attends meetings and conferences ensuring to connect with relevant professionals and others. Collaborates with others ensuring professionalism and the growth of contacts in the field.
- Prepares exhibits and makes arrangements for presentations at professional and civic meetings and events.
- Conducts interviews and corresponds with persons interested in particular phases of the program and distributes information literature and prepared materials ensuring professionalism.

A. Completed Bachelor's Degree in related field
B. Valid State driver's license
