At the direction of the Director of Quality and Compliance (DQC,) the RN will work collaboratively with the Director of Home Health, Home Health Site Managers, and members of the QAPI Committee to support Home Health QAPI activities and to meet performance improvement objectives. Is responsible for developing, implementing, maintaining, and evaluating the QAPI Program for Home Health Care. This includes monitoring patient clinical records and providing relevant staff education and coaching. Leads and evaluates performance improvement processes using best practices and established protocols to effect change. Coordinates data collection and reporting needs in accordance with clinical standards and expectations, agency policies and procedures, and applicable laws and regulations. Establishes systems to ensure the integrity of data input and reporting

Essential Duties & Responsibilities include (but are not limited to):
- Develops, implements, maintains, and evaluates an effective data driven QAPI program for home health care services. Coordinates the agenda, reports, presentations, and discussion for home health care services at QAPI meetings. Performs ongoing monitoring of the Home Health QAPI Program.
- Completes selected and random documentation review including Start of Care, recerts, and discharges, with attention paid to timely review of OASIS assessments. Conducts concurrent and retrospective chart audits.
- Compiles and submits quarterly Quality Control, Performance Improvement and Clinical Record Review reports.
- Assists Home Health Director in writing annual organizational assessment report for home health care services and reporting to home health updates to the BTBH Board of Directors.
- Graduate from an accredited school of Nursing. BS in Nursing and Public Health Certification preferred or completion of a Licensed Vocational Nursing Education Program from an accredited school
- A minimum of two years of home health or community health experience required. Quality background preferred.
- Current RN license to practice in the state of California or current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners
- OASIS Certification preferred.
