Franciscan Health Dyer Campus 24 Joliet St Dyer, Indiana 46311 The Supervisor of Inpatient Care Management is responsible for supervision, facilitation, education, and coordination of their team. In addition to the leadership of their respective team, this role is also responsible for working with assigned members, assessing, facilitating, planning, and advocating health needs on an individual basis. WHAT YOU CAN EXPECT Manage departmental finances and productivity to achieve budget targets. Assist the manager in interviewing candidates for open positions and making recommendations for hiring. Participate and lead performance Improvement initiatives; assist the director in developing and achieving benchmarks in department specific goals and metrics. Implement processes to satisfaction with those we are privileged to serve. Assist the care management team to address barriers for discharge, collaborates with patients, families and other healthcare personnel or outside agencies regarding coordination of care and discharge planning. Review documentation for completion and accuracy. Complete chart audits, meet with individual team members to share audit results and create plans for documentation improvement. Assist the manager in making decisions for direct reports in assigned function and perform people management activities such as performance evaluations, disciplinary actions, staff planning, and interviews. Supervise direct reports and manage the performance of individuals through ongoing coaching, feedback, and development to motivate, engage and drive a high performing team.
