Hours : 8 : 00 - 5 : 00 Monday - Friday, 40 hours per week (may include some evening and weekend work)
Funding period : through 8 / 31 / 2027
The Certified Peer Support Specialist will provide comprehensive peer support services to individuals with a substance use and / or mental health diagnosis, including people who are actively using substances. The Specialist will serve on a multi-disciplinary team and will focus on providing support and connections to care in a variety of settings based on client needs. Settings could include the health department, syringe service program, jail, MOUD clinic, and community-based organizations.
Primary Responsibilities :
- Provide case management
- Conduct intakes and enrollment activities
- Develop care plans for clients
- Assist team with daily data tracking and monthly reports
- Document appropriately in electronic health record
- Assist with client needs and goals
- Accompany clients to meetings and participate in meetings of community groups to advocate for client needs
Education / Experience : High school diploma or equivalent required. Certified as a Peer Support Specialist or currently working toward certification as a Peer Support Specialist by the governing agency required. Prior case management experience preferred.
License : Valid NC driver's license
