Hours : 8 : 00 - 5 : 00 Monday - Friday, 40 hours per week
The Clinical Program Coordinator provides support to clinical services operations and compliance through data management and quality improvement tasks. This position will serve as liaison between the patient support team, medical providers, and external stakeholders including insurance providers and the clinically integrated network (CIN).
Primary Responsibilities :
- Review clinical quality data and identify solutions for ongoing improvement in patient care gaps
- Run reports and organize data for the Patient Support team to make outreach to patients in an effort to close care gaps and increase access to care
- Assist with data entry and reporting of Health Center program metrics
- Assist with digital check-in development and implementation
- Maintain knowledge of Medicaid Adult and Child Health Care Quality Measures, NC Medicaid, and PHP's
- Attend monthly meetings with CCPN and Prepaid Health Plans to discuss quality, clinical updates, and new initiatives
- Provide quality data reporting to Medical Director, Clinical Directors, and Health Center QI Committee
Education / Experience : Bachelor's degree in Public Health, Nursing, Healthcare Administration or relevant field. Experience in healthcare informatics / data, quality improvement, and / or clinical services preferred. Experience with Microsoft Office Excel.
