Job Title: Resource Care Navigator

Salary Range: $50,000 - $65,000 base annual
Summary:
Cabin Creek Health Systems (CCHS) is a Federally Qualified Health Center (FQHC), Patient Centered Medical Home, with six integrated health centers, and five School Based Health centers, and is continually planning expansions and new program developments. CCHS offers many services including primary care, dentistry, 340B pharmacy programs, behavioral health, robust substance use treatment, pulmonary rehab, and many more.
The purpose of this position is to promote timely access and assignment of care for persons served by CCHS, to support stability for patients with complex needs associated to Social Drivers of Health (SDoH), and to assist them in navigating CCHS programs and services offered by partners. Furthermore, this position should act as a liaison between CCHS and the social service providers in the community. ensuring integrity to the patient across the partner agencies and the services CCHS serves.
The position will have system-wide responsibilities and will involve collaboration with CCHS provider teams in addition to available services in the community.
TARGET POPULATIONS
· Provide person-centered care to all patients including those with serious mental illness, who are unhoused or underhoused, are experiencing substance use disorder, or in some other way have high barriers to accessing healthcare in traditional ways.
DUTIES AND RESPONSIBILITIES
· Facilitate access to CCHS-based and community-based services by collaborating with CCHS provider teams, specialists within system, and other care teams across CCHS.
· Collaborate with, and facilitate patients access to necessary social services as able and appropriate while maintaining integrity to person-centered care and patient autonomy.
· Routinely consult/collaborate with CCHS administration, community partners, and applicable research partners to improve patient access and program improvements.
· Remain appraised of the resources in our communities (and assist in the awareness and utilization of these services).
· Work with other CCHS Care Navigators to maintain awareness of access and applicability of community programs. (e.g. service availability, requirements for access, insurance accepted as applicable)
· Work with care teams and patients in navigating resources to resolve barriers to care and wellness that strive to improve social determinants of health.
· Contact, and assist, patients in overcoming internal or external barriers to appropriate care.
· Contact patients between visits to discuss the patient’s status and needs, provide motivational support to the patients in following the care plan developed by the care team, and community partner as applicable.
· Triage patients need in collaboration with care teams to decrease tension on distress tolerance of the patient while minimizing burden to providers.
· Attend system and community meetings as appropriate to disseminate CCHS information to the community, and community information to the CCHS system. (e.g. act as a representative and liaison of CCHS to the community)
· Must present or disseminate information related to their work internally, to partners, or to others in small groups or at meeting and conferences.
· Other duties as assigned.
QUALIFICATIONS, JOB SPCIFICS & REQUIRMENTS
· A bachelor’s degree in social work or applicable field is required, with a masters being preferred.
· This position will work a five-day work week for eight hours a day; totaling a 40-hour work week. Some overtime may be available but would be atypical to the schedule.
· This position requires independent transportation. Staff will be expected to travel across the county in the typical execution of their job tasks with milage being reimbursed at the prevailing state rate.
· Regional or national travel is possible, but atypical and funded as needed or by arrangement.
· Must be able to stand or sit for prolonged periods of time and lift materials appropriate to complete job duties.
