What You Will Do
The Social Worker Director holds a vital role in our facility by working with residents to identify their psychosocial, mental and emotional needs. As the Social Worker Director, you will work with the resident and families to assist in providing, ensure they have access to the services to meet their needs. We’re excited for you to come and join our accomplished team of medical professionals!
Responsibilities
- Participate in reviewing and setting policies concerning resident care and quality of life
- Document progress in meeting the psychosocial needs of residents
- Schedule and participate in resident care plan and assessment meetings
- Advocate for social services that are central to improving the quality of life of residents
- Work with residents, families, significant others and staff to provide support and improve the quality of life and care for residents
- Complete a social history and psychosocial assessment for each resident that identifies social emotional and psychological needs
- Provide therapeutic interventions to assist residents in coping with their transition and adjustment to the facility
- Provide support and education to residents and family members to assist in their understanding of placement and facility issues
- Provide support and education to residents and their family members to assist in the in their understanding of placement in the facility.
Requirements
- Must possess either a Masters of Social Work or a BSW that is Licensed Social Worker in Maryland
- Must possess at least 3 years of previous Social Work experience
- At least 1 year of Long Term Care experience is required
