Duties and Responsibilities
The Chronic Care Management nurse will assist with Population Health Management as well as
calling patients weekly, reviewing medications and patient goals.
- Responsible for scheduling patients, managing the list of patients and performing billing
for the visits.
As necessary, assists with the following :
signs, chief complaint, and ensuring completion of appropriate intake forms.
is able to assist the physician or nurse practitioner as needed in the execution of those
procedures including but not limited to gynecologic procedures, joint injections, minor surgical
procedures, and suturing
scheduling appointments as needed.
examination room (manages inventory)
the physician or nurse practitioner.
medical goods forms, FMLA paperwork, etc
Job is located in the Centreville Office.
