About the Role:
Join Callen-Lorde Community Health Center, a leading LGBTQ-focused health center, as a Case Manager. We are seeking a compassionate and dedicated professional to support our diverse patient population in accessing comprehensive healthcare services.
Key Responsibilities:
As a Case Manager, your responsibilities will include:
- Conduct comprehensive assessments of patients' needs, creating individualized care plans.
- Collaborate with healthcare providers and community resources to ensure a holistic approach to patient care.
- Advocate for patients' rights and coordinate access to LGBTQ-affirming services.
- Facilitate support groups and educational sessions on relevant health topics.
- Maintain accurate and confidential case records, ensuring compliance with HIPAA regulations.
- Provide crisis intervention and emotional support to patients facing challenging situations.
- Collaborate with multidisciplinary teams to enhance patient outcomes.
Work Schedule:
This is a full-time position with evening hours.

Qualifications
- High school diploma or GED preferred.
- In the absence of education credentials, experience in social services, case management, and/or health care will be considered.
- Familiarity with LGBTQIA, HIV and health-related issues and community resources.
- Strong computer competency, including knowledge of Electronic Medical Records, Computerized Appointment/Billing Programs and other Microsoft Office Programs.
- Strong customer service orientation.
