Job Title:
LTSS Service Care Manager
Location: Remote – North Carolina (Preferred locations: Fayetteville, Wilmington, Jacksonville, Goldsboro)
Assignment Duration: 6 months, with potential to extend or convert
Work Schedule: Monday – Friday, 8:00 AM – 5:00 PM EST
Travel: Up to 80% travel within a 2.5–3-hour radius from home residence. Mileage and lodging reimbursed.

FTE Conversion Salary Band: $68,000 - $70,000
Position Summary:
The LTSS Service Care Manager is responsible for managing a caseload of healthcare members with long-term care needs. This position requires a high level of autonomy and fieldwork, conducting in-home visits, assessments, and ongoing care coordination. The role ensures members receive timely, cost-effective, and comprehensive services, while supporting their choice to remain in the least restrictive environment possible.
Key Responsibilities:
- Complete in-person assessments with members, caregivers, or providers to obtain clinical and psychosocial information.
- Develop individualized care plans tailored to each member’s needs and goals.
- Monitor service delivery and member progress through both telephonic and in-person follow-ups.
- Authorize, coordinate, and track services and referrals to ensure no gaps in care.
- Collaborate with healthcare providers, discharge planners, and interdisciplinary teams to coordinate services across care settings.
- Support members with appeals, grievances, and navigation of long-term services and supports.
- Maintain accurate and timely documentation in electronic health records.
- Comply with state, federal, and Centene regulations and standards.
Required Qualifications:
- Education: Bachelor’s degree
- Licensure:
- Required: Valid RN or LCSW license
- Required: Valid driver’s license (must be willing to travel up to 80%)
- Experience:
- Minimum 2 years of care management or case management experience
- Background in physical health, home health, utilization management, or discharge planning
- Technology Proficiency:
- Experience with Microsoft Office and electronic medical records (EMR)
Preferred Qualifications:
- 4–6 years of total care management experience
- Experience in long-term care, home health, or discharge planning
- Familiarity with Medicaid or managed care organizations
- Strong documentation, time management, and organizational skills
