Job Title: Care Manager
Location:
- Worksite: 75-90% remote
- Specific Location Requirements: Hillsborough and Polk County
- Field Work: Yes, member-facing
- Schedule: 8:00 AM - 5:00 PM, Monday through Friday
Key Responsibilities:
- Perform daily outreach calls to members based on tier-specific needs.
- Address barriers to care, check in on members, and follow up on any specific needs.
- Depending on members’ needs, manage a call volume ranging from 5–20 calls/day.
- Conduct care management tasks, including assessing, planning, and coordinating medical and supporting services across the continuum of care.
- Facilitate face-to-face visits if required based on member tiers.
- Support members in accessing community resources related to housing, employment, independent living, foster care, and justice system needs.
- Document all interactions and complete required care plans, annual assessments, and other relevant tasks in a timely and accurate manner.
Performance Metrics:
- Monthly audits for compliance with care management policies and procedures.
- Key Performance Indicators (KPIs) include meeting monthly/quarterly outreach goals, completing care plans, and ensuring timely documentation.
- Completion of annual assessments and face-to-face visits as required.
Education Requirements:
- Required: Bachelor’s Degree in a related field.
Experience Requirements:
- Required:
1. 2 years of pediatric case management experience.
2. Behavioral health experience.
3. Experience connecting families and children to local resources (housing, foster care, justice, etc.).
- Preferred:
1. Health plan experience.
2. Foster care and/or .
3. Bi-lingual abilities (especially or Creole).
Licenses/Certifications:
- Required:
- Social Work Licensure (Licensed within the State of Florida or Compact Licensure).
Software Skills:
- Proficient in Microsoft Office Suite (Outlook, Word, Excel).
Onboarding and Training:
- First day includes setup of equipment, onboarding tasks, and review of training schedules.
- Training duration: 4 weeks.
Ideal Candidates:
- Passionate about supporting individuals with behavioral and mental health challenges.
