Role Description
Cambay Healthcare, LLC is seeking a full-time, on-site Director of Case Management located in Exeter, NH. The Director of Case Management will oversee discharge planning, utilization management, medical case management, and Medicare processes. This leadership role requires a strong focus on managing and guiding teams to ensure the best care and outcomes for patients.
Qualifications
- Expertise in Discharge Planning and Utilization Management
- In-depth knowledge of Medical Case Management
- Experience with Medicare processes
- Strong leadership and organizational skills
- Excellent communication and interpersonal abilities
- Ability to thrive in a fast-paced environment
- Bachelor's degree in Nursing or a related field
- Case Management certification is a plus
Requirements
- 7-10 years of related experience (MUST have Case Management Managerial/Supervisory/Director experience) – Required
- BSN or a combination of BSN and MSN – Required
- Hospital/Medical Center experience is essential
- Active NH license required
- Housing should be researched and/or confirmed prior to accepting the role

- Case Management skills checklist/related competency exam
- Supervisor references from within the last 3 years (2 separate references required)
- Active license and certificates
- BLS (AHA)
