Job Summary:
The Leadership Director of Case Management is responsible for overseeing and coordinating case management services within an organization. This role involves organizing and directing case managers, developing and implementing case management policies and procedures, and ensuring the efficient and effective delivery of services to clients.
Responsibilities and Duties:
- Provide leadership and guidance to a team of case managers
- Develop and implement case management policies and procedures
- Monitor and evaluate the quality and effectiveness of case management services
- Collaborate with other departments and stakeholders to ensure coordinated care for clients
- Develop and maintain relationships with community resources and service providers
- Ensure compliance with regulations and standards related to case management services
- Provide training and support to case managers to ensure their professional development
- Manage budget and resources allocated for case management services
Qualifications and Skills:
- Bachelor's degree in social work, psychology, nursing, or a related field (Master's degree preferred)
- Certified Case Manager (CCM) certification preferred
- Minimum of 5 years of experience in case management, with at least 2 years in a leadership role
- Strong leadership and communication skills
- Knowledge of legal and ethical issues related to case management
- Ability to develop and implement policies and procedures
- Experience in working with diverse populations
- Proficiency in using case management software
