Job Summary : The Director of Case Management Resource is responsible for overseeing and coordinating all aspects of case management within skilled nursing facilities. This role ensures that residents receive comprehensive, high-quality care tailored to their individual needs. The Director will lead a team of case managers, collaborate with healthcare providers, and develop strategies to improve patient outcomes and operational efficiency.
Essential Duties and Responsibilities
- Leadership and Management :
- Lead and manage the case management team, providing guidance, support, and professional development opportunities.
- Develop and implement policies and procedures to ensure effective case management practices.
- Monitor and evaluate the performance of case management staff, ensuring compliance with regulatory standards and best practices.
- Patient Care Coordination :
- Oversee the assessment, planning, implementation, and evaluation of patient care plans.
- Ensure that all residents receive appropriate and timely care, including medical, social, and psychological services.
- Coordinate with interdisciplinary teams to facilitate comprehensive care and discharge planning.
- Quality Improvement :
- Identify areas for improvement in case management processes and implement strategies to enhance patient care and operational efficiency.
- Monitor patient outcomes and satisfaction, using data to drive quality improvement initiatives.
- Ensure compliance with all federal, state, and local regulations related to case management and patient care.
- Monitor and improve payer authorization capture, to support patient needs and reduce hospital readmissions
- Collaboration and Communication :
- Serve as a liaison between residents, families, healthcare providers, and external agencies.
- Attend monthly meeting with health plans
- Foster strong relationships with community resources to support residents' needs.
- Communicate effectively with all stakeholders to ensure a seamless continuum of care.
- Budget and Resource Management :
- Develop and manage the case management budget, ensuring efficient use of resources.
- Identify and secure funding opportunities to support case management programs and initiatives.
