GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
The PASRR habilitation coordinator position is responsible for service planning, monitoring, assessing, and identifying needs and desires of consumers diagnosed with intellectual and developmental disabilities relating to personal outcomes, including recommendations for interventions and support services. Arranges and collaborates with service . Arranges and collaborates with service providers and nursing facilities to link consumers with outcome-focused services. The habilitation coordinator will be responsible for managing a caseload not exceeding forty (40) people. The caseload will consist of consumers who reside in a nursing facility or are at risk of being admitted into a nursing facility and utilize the Preadmission Screening and Resident Review (PASRR) funding stream. The habilitation coordinator is responsible for making contracts with assigned individuals in accordance with their service coordination assessment. Habilitation coordinators are responsible for completing annual and quarterly Service Planning Team (SPT) meetings for each PASRR person on their caseload and completing timely and accurate documentation of the delivery of habilitation coordination services. Requires flexibility in scheduling and significant travel within the service area. The job calls for a self-motivated and energetic person to assess consumer’s outcomes and needs and provide monitoring of services to consumers living in nursing homes. The caseload will be primarily concentrated in Atascosa, and Frio counties. Habilitation coordinators are required to meet set productivity standards monthly. This position will be housed in Dilley, Texas and will be directly supervised by the Director of PASRR housed in Dilley, Texas.
MINIMUM JOB QUALIFICATIONS:
· Bachelor’s or Master’s degree from an accredited college or university with a major in social, behavioral health or human services field.
· Must have one (1) year of habilitation/service coordination experience in providing service coordination to individuals with IDD.
· Must possess proficient computer skills.
PREFERRED JOB QUALIFICATIONS:
· Bi-lingual in English and Spanish.
· Experience working with individuals residing in a nursing facility.
· Prior work experience as a Service Coordinator or Case Manager working with people with intellectual or development disabilities.
