• Develops and implements individualized plans of care for patients
• Coordinates care with other healthcare professionals to ensure seamless transitions and optimal patient outcomes
• Communicates effectively with patients, families, and healthcare team members to promote collaborative care
• Maintains accurate and detailed records of patient care activities and outcomes
Qualifications
• Bachelor's degree in Nursing from an accredited institution
• Current Wyoming LPN license
• One year or more of experience in Medical/Surgical and Psychiatric Nursing preferred
