What are the responsibilities and job description for the MFIP Case Manager position at CAPI USA?
The MFIP Case Manager is responsible for providing intensive case management, career and employment counseling, and coordination of support services to a revolving caseload of 60-80 participants in the Minnesota Family Investment Program (MFIP). This position connects participants to linguistically and culturally appropriate education and employment activities as well as to community resources, towards the goal of helping participants achieve self-sufficiency through employment.
WAGES AND BENEFITS: $21.00 per hour; medical insurance, including generous employer contributions to a Health Savings Account; dental and vision insurance; participation in the agency-sponsored retirement plan with a 3% match; employer-paid life insurance; short- and long-term disability insurance; generous paid time off, plus 10 employer paid holidays and 1 floating holiday per year.
SCHEDULE: The majority of this position is conducted 8:30am - 4:30pm, Monday through Friday. The schedule is currently hybrid with two days per week in the office, and will eventually transition to 3-4 days per week in office.
JOB RESPONSIBILITIES:
Enrollment and Assessment
· Enroll participants and complete appropriate intake process according to MFIP and agency policies.
· Conduct initial and ongoing assessments per MFIP and agency policy.
· Determine participant eligibility through formal and informal inquiry and screening of participants.
· Provide program information to participants individually and/or in a group setting.
· Promote other CAPI services and represent the agency positively; screening for eligibility and interest in other CAPI programs and services, and completing referrals as appropriate in collaboration with other CAPI staff.
· Initiate and support individualized exploration of career/educational paths available within MFIP.
· Engage participants in the program and proactively initiate productive communication.
Case Management
· Maintain compliance with MFIP policy and related contract rules and requirements.
· Provide intensive, person-centered, participant-led case management and supportive employment counseling to individuals of diverse cultural, economic, and educational backgrounds who may have multiple barriers to employment.
· Support and coach participants to define individualized goals and to identify a path to economic self-sufficiency.
· Work collaboratively with participants to develop an appropriate Employment Plan with specific activities, realistic timelines, and measurable outcomes.
· Provide ongoing coaching and person-centered employment counseling once an Employment Plan is developed.
· Collaborate with Hennepin County staff, including Childcare Assistance and eligibility workers, to manage cases appropriately and coordinate benefits.
· Refer participants to external resources and assist them with access; collaborate with external service providers, including but not limited to schools, legal advocates, housing programs, and mental and physical health practitioners.
· Collaborate with CAPI’s Workforce staff, including CAPI’s Employment Navigator, and with external parties (such as employers and training providers) to facilitate job placements for participants.
· Monitor, evaluate, and report on individuals’ progress on an ongoing basis.
· Assist participants in completing request processes and obtaining verifications necessary to receive MFIP support services.
· Provide interpretation resources to participants with limited English proficiency.
Data/File Management and Reporting
· Meet outcomes defined by MFIP contract and contribute to department-wide performance.
· Maintain proper documentation in physical case files per MFIP and agency requirements.
· Input required data into databases, including Workforce One and Client Track, in an accurate and timely manner.
· Maintain case notes in Workforce One, documenting communications, actions taken, and other relevant information.
· Participate in periodic internal and external case file audits.
Other
· Serve as a strong team player, supporting CAPI’s mission and strategic imperatives.
· Complete internal and external trainings as required.
· Attend and actively participate in team, department, and all-staff meetings.
· Other duties, responsibilities, and projects as assigned.
REQUIRED QUALIFICATIONS:
Education and Experience:
· Bachelor’s degree in Social Work, Psychology, or a related human services field required; OR a combination of demonstrably equivalent education and experience.
· Demonstrated experience with diverse populations preferred; ability to work effectively with diverse populations required.
· Two years of professional experience in MFIP case management, employment/vocational counseling, or other human services area preferred.
Knowledge, Skills and Abilities:
· Ability to complete work with attention to detail.
· Excellent interpersonal communication, counseling/coaching, and conflict resolution skills.
· Bilingual communication skills, especially Somali or Spanish languages preferred; ability to proficiently speak/write/read/understand English required.
· Ability to work effectively in a mission-driven agency whose clients and staff exhibit significant diversity with respect to race, ethnicity, gender orientation, socio-economic status, nationality, and religion.
· Ability to balance competing priorities, shift focus quickly, and adhere to deadlines.
· Ability to exercise sound and rational judgement when assisting participants with career and self-sufficiency planning.
· Ability to learn and use a variety of computer programs competently to perform daily work.
· Knowledge of systemic social and economic challenges faced by people experiencing poverty preferred.
· Knowledge of local service agencies and community resources preferred.
· Knowledge of MFIP program preferred.
Licenses and Certifications:
· Global Career Development Facilitator credential preferred; or ability to obtain within first year of employment while performing essential functions of the job.
· Valid MN Drivers’ License, access to reliable transportation, ability to meet CAPI’s driving record requirements, and ability to transport participants required.
COVID protocols: CAPI USA requires employees to be vaccinated as a condition of employment, subject to reasonable accommodation required by law. All vaccination doses must have been received at least fourteen days prior to starting employment. CAPI requires and provides masks, social distancing, sanitizes common services regularly and is currently open by appointment for most services. As roles and responsibilities allow, most staff are working a hybrid schedule with some days in the office and some days work from home to allow for appropriate social distancing while working at CAPI sites. Currently staff are working two days per week in the office and three days per week from home, which is subject to change based on CAPI’s protocols, the needs of specific roles, and the needs of those that CAPI serves.
