General Summary:
Position is responsible for the following functions of the Community Living Assistance & Support Services (CLASS) program:
- Provide required documentation to HHSC as necessary to assess and renew the level of care for the individual.
- Must submit annual assessments data to HHSC Portal and monitor for approval
- Assist in developing a habilitation plan to outline the individual's habilitation needs and complete documentation of that plan.
- Participate in developing an Individual Plan of Care (IPC) that addresses all of the individual's needs that will be met through the provision of CLASS and CFC services. provide all CLASS and CFC provider-managed services according to the Individual Program Plan (IPP);
- Monitor the DSA's service provision processes to ensure services are delivered by qualified service providers in accordance with the IPP
- Coordinate with the Case Management Agency (CMA) and other service providers as necessary to ensure IPC revisions are initiated as necessary in response to changes in the individual's needs.
- Monitor and maintain monthly MESAV for each participating individual.
- Monitors and maintain all paper flow for each service provided for each individual participating in the CLASS program.
The CLASS Coordinator promotes health, safety, community participation and desired lifestyle by ensuring services are provided as authorized on the Individual Plan of Care. Position is responsible for ensuring compliance of the agency with the CLASS program regulations and requirements, as well as state and federal regulations. Position is responsible for hiring attendants, contracting with therapy service providers, minor home contractors, and supervising and training all supporting staff in the provision of services, documentation, recordkeeping, and billing. Responsible for searching for non-waiver resources and utilizing them prior to requesting CLASS funds. CLASS Coordinators documents and provides oversight of the documentation of all services as required, assures services are billed timely, and assures compliance of personnel with the policies and objectives of the Agency. Position can be called upon to perform other duties as assigned.
Job Skills and Abilities:
Must have the ability to follow instructions; must be able to speak and understand the English language. Ability to learn, manage, and ensure regulatory compliance of various health programs. Familiar with Microsoft Office applications including Word and Excel. Strong written and oral communication skills required. Requires a valid Texas driver’s license.
Qualifications:
Education and/or experience equivalent to graduation from High School or GED equivalent. Position requires at least one (1) year of administrative support experience. Prefer experience in the home health field.
