POSITION SUMMARY:
Assist, coordinate, and treat a case load of patients with co-morbid conditions of mental/substance use disorders at a nurse-led rural federally qualified health clinic that uses a collaborative integrated health care model. This position will collaborate with the Behavioral Health Director, clinical psychiatrist, physicians, nurse practitioners, and office staff at all Capstone Health locations. The manager will serve as a liaison between clinic staff, other behavioral health providers, and community services to provide comprehensive interdisciplinary mental health/substance abuse treatment for rural clinics. The salary range for this position is $58,500 to $70,000.

Relationships
- Develop favorable relationships with all patients. Instruct patients and their family members training and /or information related to connection with prevention and awareness of substance abuse/mental health.
- Establish favorable working relationships with all staff members associated with center operations, including nurses, physicians and related staff.

PRIMARY TASKS & DUTIES:
- Develops, implements, and evaluates plans for the provision of mental health/SUD and support services jointly with the Psychiatrist, Medical Providers, and other staff members as appropriate.
- Performs comprehensive bio-psychosocial assessment for mental health, substance abuse, co-occurring disorders, and medical needs. Conducts follow up as appropriate based on results of evaluation.
- Address referrals from providers including referrals via warm hand-offs.
- Establishes and maintain a current listing of community linkages to meet the needs of the target population. Initiate linkages, disseminate information and make referrals to the appropriate services as needed.
- Must be knowledgeable in cultural diversity and ability to work well with patients with multiple problems.
- In-depth knowledge of drugs of abuse, routes of use, symptoms of intoxication and withdrawals.
- Knowledge of substance abuse/dependence disorders, substance-induced disorders and appropriate levels of treatment.
- Must be knowledgeable of issues and therapy techniques relating to substance use & dependence as well as issues relating to mental illness.
- Participates in regular interdisciplinary team meetings as scheduled.
- Meets regularly with the immediate supervisor (Behavioral Health Director) as a means of enhancing professional growth, reviewing and processing care management services, and dealing with administrative responsibilities.
- Maintain client records, including all mandated agency forms in accordance with agency policies, state confidentiality regarding privileged administrative and client information in a professional manner.
- Documents correspondence and contracts in a timely manner and maintains case records per Capstone Health policy.
- Assists with office duties as assigned.
- Other duties as assigned.
ESSENTIAL FUNCTIONS/KEY COMPETENCIES:
· Read, write, and maintain patient records and related administrative documentation
· Knowledge and ability to use requisite tools, systems, technology and equipment in the collection of patient data
· Utilize computers in the collection, management, and reporting of patient records
· Ability to work with a diverse, sometimes difficult and demanding patient population
· Adherence to risk management and quality programs.
POSITION REQUIREMENTS:
Education
- Qualifications for the position include a history of superior management and leadership plus a strong desire to align with the mission of Capstone Health. The ideal candidate will have a minimum of a master’s degree and hold a license as a Clinical Social Worker, Licensed Professional Counselor, Licensed Family and Marriage Therapist or Psychologist.Mustpossess and maintain a full unrestricted license to practice in the state of Alabama. Must have knowledge of serious mental illness, mental health assessment, consumers’ rights, drug abuse/addiction, and local community resources. Experience working in a primary care setting is preferred but not required. This position requires the ability to effectively interact and collaborate with interdisciplinary team members and rural community members.
