Cardinal Hayes Home for Children for Children is seeking a Day Habilitation Coordinator/ QIDP to join our caring and compassionate nursing team in Millbrook, NY and surrounding areas.
We are a non -profit agency in Millbrook, NY responsible for ensuring that all residents are cared for, that their needs are understood, that their preferences are followed, and that their rights are protected.
Schedule- Monday through Friday 8am-4pm (35 Hours per week)
Job Overview
The Day Habilitation Coordinator is responsible to plan, organize, implement, and oversee the day to day operations, activities and services of the Cardinal Hayes Without Walls (WOW) Program. This is a “hands-on” leadership position which provides supervision, assistance, guidance, and training to both the participants receiving services, as well as the Day Habilitation Specialist and Direct Support Professionals assigned to the program.
Responsibilities
- Use Person-Centered Planning practices and Quality of Life standards to plan and organize a wide variety of individual and group activities daily, which actively engage participants in areas of their interest.
- Review and certify Habilitation Plans and Staff Action Plans and support the Habilitation Specialist to ensure that services are implemented as written and reviewed in accordance with regulations.
- Periodically review data collection and assist Habilitation Specialist, as needed, to ensure that documentation of each individual’s services is accurate, complete, and recorded in a timely manner.
- Review and certify monthly notes and billing accuracy in accordance with regulations, while working in conjunction with the Cardinal Hayes Fiscal Department.
- Maintain and review accurate attendance for each individual.
- Ensure that the health and safety of program participants and personnel is maintained at all times throughout the program day in program areas, vehicles, and in the community.
- Manage internal systems. This includes maintaining adequate staffing, completing payroll, maintaining an attendance controller for staff, managing petty cash and small amounts of client funds, creating purchase orders, and various other duties.
- Oversee the WOW Program’s vehicle fleet, which includes working with the Fleet Specialist to ensure yearly inspections and general maintenance. Provide program oversight of monthly and daily inspection documentation, cleaning records, and gas cards. Ensure vehicles are maintained and operating safely.
- Ensure that the physical care and supervision of program participants is provided in a manner which respects each individual’s dignity and promotes independence. Monitor and report any health concerns to the Medical Department.
- Follow agency procedures for the safe handling of food and ensure procedures are in place to adhere to the participant’s prescribed dietary needs
- Report immediately any situation or circumstance that could potentially jeopardize the health, safety, well-being, or rights of any individual in the program, following regulatory guidelines.
- Ensure completion, and review required documentation of incidents, accidents, etc.
- Complete required agency trainings. Monitor and ensure that staff complete their required trainings and certification updates annually.
- Mentor, supervise and evaluate staff performance as required.
- Actively model the Cardinal Hayes Values of Respect, Dignity and Compassion toward all individuals.
Qualifications and Education Requirements:
- Bachelor’s Degree in Human Service or related field
- Minimum of one year experience working with people with ID/DD
- Valid NY State Driver’s License is required
- Must demonstrate ability to drive company vehicles including Minibus and Transit Van
- Proficiency with computer applications including but not limited to Microsoft Office
- Knowledge of Precision Care and OPWDD CHOICES, preferred
