Job Summary
The Medical Case Coordinator acts as a liaison for Care Connect Medical and our network of clinics to provide an effortless experience for patients. By utilizing pristine phone etiquette, and clear written and verbal skills, the Medical Case Coordinator will ensure a smooth flow of communication between patients and providers. This role requires an individual who is highly organized, able to escalate prioritized issues, and can report on patient progress. They will also perform a variety of clerical, recordkeeping, and customer service tasks to assist with the smooth operation of the organization.
Duties/Responsibilities
- Generates, monitors, and analyzes daily reports.
- Reviews new incoming patient referrals and treatment history.
- Ensures smooth and efficient scheduling of patients.
- Communicates professionally and effectively with patients, providers, and colleagues.
- Fields incoming department calls and transfers to applicable parties as needed.
- Notifies management of any existing or potential errors or concerns within CRM.
- Records all pertinent interactions in CRM.
- Reviews and analyzes new information or referrals submitted by providers.
- Communicates with patients in a professional and caring manner.
- Escalates questions and concerns to team Lead in a timely manner.
- Monitors and assists with patient compliance issues.
- Acts as a liaison between medical providers and the patient.
- Ensures work is conducted in compliance with HIPAA.
- Completes other projects and duties as assigned.

- Basic understanding of office equipment.
- Basic understanding of clerical procedures and systems such as recordkeeping and filing.
Education and Experience
- High School Diploma or GED required.
- Associate or Bachelor’s degree preferred, but not required.
- 5 years of case management or medical administrative work preferred.
- Experience in hospitality or customer service environments preferred.
- Bilingual preferred: English and Spanish.
Physical Requirements
- Prolonged periods sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at times.
Salary : $60,000 - $70,000
