Job title: Case Management Director (Hospital Case Manager, Director)
Job Summary:
The Case Management Director is responsible for overseeing and coordinating the daily operations of the hospital's case management department. This includes managing a team of case managers, ensuring quality patient care, and working closely with healthcare providers to develop and implement individualized care plans for patients.
Responsibilities and Duties:
1. Develop and implement processes to improve patient outcomes and reduce hospital readmissions
2. Oversee the assessment, planning, implementation, and evaluation of patient care plans
3. Coordinate with nurses, doctors, social workers, and other healthcare professionals to ensure effective communication and collaboration
4. Monitor and analyze data to identify trends and areas for improvement
5. Provide leadership and guidance to case management staff
6. Ensure compliance with state and federal regulations and accreditation standards
7. Collaborate with insurance companies and other payers to facilitate timely discharge planning
8. Conduct regular staff meetings and training sessions to promote professional development
9. Serve as a liaison between patients, families, and healthcare providers to address concerns and facilitate communication
10. Participate in quality improvement initiatives and committees within the hospital
Qualifications and Skills:
1. Bachelor's or Master's degree in nursing, social work, or a related field
2. Current RN or social work licensure (preferred)
3. Minimum of 5 years of experience in case management, with at least 2 years in a supervisory role
4. Strong leadership and communication skills
5. Knowledge of healthcare laws and regulations
6. Ability to analyze data and identify areas for improvement
7. Experience with electronic health records and case management software
8. Excellent organizational and time management skills
9. Ability to work collaboratively with a diverse group of professionals
10. Certification in case management (preferred)
