Description
This employee will work as part of the PCMH team with the following essential duties and responsibilities:
- Provide clinical supervision for new disease / care management initiatives across the CFH system of care
- Supervision and evaluation of all health center nursing and medical assistant team members
- Implement changes to policies and procedures
- Supervision of vaccine stock maintenance
- Oversee timely locking of nursing charts
- Collaborate with Director of Nursing to educate staff on annual vaccination requirements for health care workers
- Day to day management of nursing responsibilities and quality management of emergency response equipment such as banyan box and AED
- Oversee medication stock and maintain site inventory including proper discarding of expired medication.
- Actively participates in the expansion of the telemedicine and virtual visit patient access model
- Collaborate with PCP in the disease management model addressing condition-specific patient education needs
- Be able to travel across multiple sites (Bronx, Brooklyn, Queens, and Manhattan) as needed
- Be able to work weekends based on organizational need for staff coverage
- Oversee the highest standard of care for the service delivery of the following:
- Reach out and engage patients with respect to their health status, substance
- abuse and social service problems;
- Assess the physical and psychosocial needs of the patient;
- Take a complete history of individual and family members;
- Make referrals to primary care provider for chronic or episodic care;
- Administer immunizations, tuberculosis tests, monitor medication;
- Advise and educate clients about their physical and mental health, self-care, prevention
- strategies, and use of medical resources;
- Work in nontraditional environments, e.g., non-clinic settings.
- Participate in scheduling and coverage needs across the health center network
- Provide direct care to support clinical staff when patient cycles are backed up and/or clinical teams are short staffed
- Ensure proper documentation and administration of vaccines in accordance with VFC and AFC standards
- Collaborate with Director of Nursing to ensure that health centers have adequate amounts of medical supplies and oversee appropriate stock maintenance
- Oversee, supervise, and train all point of care testing (e.g. HIV, Hemoglobin A1C, pregnancy tests, and EKGs) and other onsite diagnostic procedures, ensuring proper documentation including billing
- Work collaboratively with case management and health education to effectively coordinate care as directed by the primary care medical staff
- Provide clinical education and supervision for medical assistant staff
- Maintain high standards of infection control and appropriate disposal of sharps and other hazardous materials
- Oversight of all quality logs such as refrigerators and freezers
Qualifications
JOB QUALIFICATIONS:
- Master’s degree in clinical or human services field from an accredited professional school.
- Clinical Training (RN). Five years of direct patient/ client care
- Three years of supervisory/ management experience working in an ambulatory health care facility.
Experience:
- 3-5 years in senior management /and field experience in the development and operation of a diversified, decentralized health care facility.
- Commitment to vulnerable and disadvantaged populations.
- Flexible for hours needed and able to work in a nontraditional setting (shelter).
Skill Sets:
- Refined organizational skills and leadership qualities, excellent communication skills
- Knowledge of health care planning and administration within an ambulatory health setting serving a multi-problem clientele.
- Ability to supervise a diverse staff of professionals and paraprofessionals in a multi-service setting.
- Ability to represent the organization in community forums and educational seminars.
- Excellent written and verbal communication skills; conflict resolution skills, demonstrated commitment to staff and personal continuing education and training to improve the ability to deliver superior service for the population served by CFH.
- Proficient computer skills. Usage of EMRs.
- Ability to use email and Teams, and texting as modalities of communication daily.
