Job Description

Position Summary:

Care Hawaii is a behavioral health care organization seeking a compassionate and dependable Support Worker to support the daily functions of CAREs provision of services.

CARE Hawaii, Inc. contracts with multiple stakeholders to provide an array of services.

The Support Worker is a member of a multidisciplinary team, responsible for organizing, coordinating, monitoring all contract operations under the direction of the Director.

The primary duty of the Support Worker is to support CCS members and to pro-actively engage high-acuity members. This member may work in collaboration with other providers and any other members of the consumer’s circle of support.

Position Qualifications:

  • Minimum Education: HS diploma or GED or Associates degree. Preferred at least twelve (12) hours earned college credit hours in human services/social services subject OR b) at least forty (40) hours of continuing education in human/social services subject. The continuing education may consist of seminars, conferences, adult education courses, certification coursework, employer training or similar educational activities. Required References: Two verified professional references attesting to the individual's skills, ability & competency to meet the position duties and responsibilities.
  • Minimum Experience: Clinical skill and experience to assess and provide stabilization and support for family members and clients at risk for suicide/homicide, or experiencing other crisis situations
  • Required Certification/Registration: o CPR and First Aid o TB o Background Check o CPI o Valid Driver’s License o Vehicle: Current and Valid Documentation

Required Skills: Must have the ability to demonstrate in excellent interpersonal skills to effectively work with all levels of personnel and with other agencies. Mature, reliable, and responsible. Must have clear, concise, and empathetic communication skills. Must be culturally sensitive and aware. Must know the basics of ethics, HIPPA standards, and problem solving. Must display sound judgment, a good work ethic, and personal integrity. Must have knowledge of community resources, outreach, mental/behavioral health issues in Hawaii. Ability to relate effectively to individuals with a mental illness, substance abuse issues, co-occurring disorders, and homelessness.

Job Description

Technical Skills: Basic use of a computer and/or tablet, Microsoft Word Suite, fax machine, telephone, and text and chat features on the telephone or computer.

Working Environment: Community-based settings; provides outreach in the community.

Indoor/outdoor, open windows or air-conditioned environment; may require some travel, over-time, weekend, and holiday work, working with other Federal and/or State accounting bodies, noise level in the environment may be moderate to noisy. Exposure to infectious diseases is a relatively common risk.

Special Demands: Ability to respond effectively to the varied needs and demands of multidisciplinary team members. Have access to helpful, adequate, competent, and continuous supports and services. Harnesses the active involvement of team members in taking ownership of aspects of program development, including operations, record keeping, staff/client education, treatment planning, and quality assurance activities.

Consumers Served: CARE Hawaii will provide services to any individual who meets the criteria of the program, eighteen (18) years or older who have a serious mental illness

(SMI). The treatment of individuals should reflect an understanding of the developmental needs and range of treatment needs for each client.

Physical Demands:

1. Sit for long periods of time, use of proper ergonomics, the ability to focus on a computer and/or tablet for long periods of time.

2. Ability to assist with the high volume of calls; ability to communicate effectively both verbally and in writing is essential.

3. Ability to follow instructions and motivation to refer to various rehabilitative groups. Frequent reading of printed materials is essential.

4. Listening skills necessary to effectively respond to consumers.

5. Ability to identify and support the daily interactions and to deal with each situation in a stress-free manner.

Duties and Responsibilities:

Essential Functions

  • Demonstrates a welcoming, empathetic, and integrated approach towards all consumers on the team
  • Support primary case manager with meeting face to face visits for high acuity consumers.
  • Responsible to meet acuities assigned to support worker.
  • Support worker needs to respond as necessary within 1.5 hours to crisis situations or Emergency Department to provide in person support.
  • Assist case manager with face to face follow up with consumers post Behavioral

Health Emergency Department visit within 72 hours.

  • Assist case manager with follow up with face to face follow up visit within 2 days of discharge from an inpatient psychiatric hospitalization.
  • Support Worker will need to be available to take on-call phone line day and night shifts as well as Support Worker must answer the line.

Job Description

  • Assist with re linking consumer with insurance benefits
  • Acts as liaison and consult with community agencies and families to maintain coordination in the crisis management process
  • Educate and support clients' families, and advocate for clients' rights and preferences
  • Document client progress to maintain a permanent record of client activity according to established methods and procedures
  • Provide ongoing assessment and response to crisis management
  • Provide transportation for clients
  • Enter progress notes within 24 hours
  • Provides required documentation in a timely manner
  • Willingly performs other duties/responsibilities as requested

Additional Duties

1. Participate in team meetings

2. Prepare written or electronic reports as requested

3. Interfaces with CARE Hawaii, Inc. staff

4. Maintain chart filing, ensuring charts are properly completed thoroughly and monitor charts to ensure charts are always compliant and audit ready
