Mental Health Kokua (MHK) is seeking a compassionate and resourceful Case Manager to join our team. This full-time role provides direct support to adults with mental illness, helping them achieve stability, independence, and a better quality of life.
Case Manager Job Description:
• Conduct behavioral health assessments and develop person-centered service plans.
• Coordinate treatment, referrals, and entitlements for consumers.
• Provide follow-up support after hospitalizations and during ongoing care.
• Collaborate with medical staff and monitor consumer progress.
• Offer transportation, budgeting guidance, and recovery-based support.
• Maintain timely and accurate clinical documentation.
• Participate in supervision and training to ensure quality services.
Case Manager Requirements:
• High school diploma required; bachelor’s degree preferred, especially in a behavioral health-related field.
• Experience supporting individuals with mental illness and co-occurring disorders is a plus.
• Strong communication, teamwork, and problem-solving skills.
• Flexible, empathetic, and ready to support others through challenges.

Compensation details: 27-28 Hourly Wage
