Case Manager Requirements:
- Bachelor's degree in psychology, sociology, social work, or related human/social services field preferred.
- One and a half (1 ½) years of specialized training in case management or human services preferred.
- Competency in providing services for dually diagnosed individuals is advantageous.
- Successful completion of a criminal background check and pre-employment drug screening.
- Possession of a valid Hawaii driver's license, vehicle insurance, and safety inspection.
- Current TB Clearance or TB X-Ray.
- Current CPR & First Aid Certificate.

Case Manager Responsibilities:
- Assessment: Conduct prompt behavioral health assessments and facilitate referrals for psychiatric and medication evaluations.
- Service Planning: Develop and revise personalized treatment plans, coordinate services and referrals, and provide ongoing support to monitor progress.
- Follow-Up: Ensure continuity of care post-hospitalization through necessary outreach services.
- Hospitalization Support: Assist consumers in preventing re-hospitalization for psychiatric care.
- Coordination: Verify insurance coverage, collaborate with healthcare providers, and coordinate efforts with fellow Case Managers.
- Adaptability: Embrace evolving responsibilities in line with organizational and industry standards.
- Recovery Model: Implement and support consumers in achieving health and wellness goals through the recovery model.
- Documentation: Maintain accurate and timely clinical records, including progress notes and required forms.
- Clinical Supervision: Participate in clinical supervision sessions to discuss consumer progress, ethical considerations, and cultural competency.
- Program Compliance: Adhere to Mental Health Kokua's case management program guidelines and ethical standards.
- Additional Responsibilities: Perform other duties as assigned by management.
