Position Summary:

Care Hawaii is a behavioral health care organization seeking a compassionate and

dependable Program Manger to support the daily functions of CAREs provision of

services. CARE Hawaii, Inc. contracts with multiple stakeholders to provide an array of

services. The Program Manger is a member of a multidisciplinary team, responsible for

organizing, coordinating, monitoring all contract operations under the direction of the

Director.

The primary duty of the Program Manger is the administration of the program. The

Program Manager is a member of the interdisciplinary team that is responsible for

supervising and directing all staff activities, education, and training according to contract

requirements and best practice guidelines. This member may work in collaboration with

other providers and any other members of the consumer’s circle of support.

Position Qualifications:

  • Minimum Education: Bachelor’s degree from a nationally accredited college or

university in one or more of the following fields:

- Social Work

- Nursing

- Counseling

- Psychology

- Psychosocial Rehabilitation

- Sociology

- Human Development

- Other closely related fields, as approved in writing by the AMHD Chief or

designee, providing the individual has completed: 12 credit hours of post high

school coursework in the areas of psychology, counseling, social work, or other

areas of human development, and one (1) year of experience providing direct

services to individuals with mental illness or other behavioral health issues.

Individuals may also qualify by having one (1) or more of the following credentials: A

Certified Psychiatric Rehabilitation Practitioner (CPRP) or any one of the following

credentials listed below in the State of Hawaii, along with 12 credit hours of post high

school coursework in the areas of psychology, counseling, social work, or other

areas of human development, and one (1) year of experience providing direct

services to individuals with mental illness or other behavioral health issues.

- A Certified Substance Abuse Counselor (CSAC)

- A Certified Peer Specialist (HCPS)

- A Registered Nurse (RN)

- A Licensed Practical Nurse (LPN)

All post high school coursework must have been completed at, and the degree

issued by a nationally accredited institution.

Job Description

For degrees issued outside of the United States, the issuing institution must meet

similar accrediting standards or be recognized within the United States as having

equal standing.

  • Minimum Experience: Strong clinical skills and experience demonstrating

competence in problem-solving, critical thinking, collaboration, and clinical

competencies in providing education and training to staff members. Prior

supervisory experience: leadership ability, demonstrating excellence in interpersonal

skills to effectively work with all levels of personnel and with other agencies. Interest

and knowledge of current trends and new developments in the mental health field

for the adult target population. Must have a minimum of two (2) years of experience

in mental health, interest and knowledge of current trends and new developments

in the mental health field for the adult target population. Inpatient/outpatient

psychiatric nursing preferred. Applicable experience will be included regardless of a

paid or unpaid experience.

o CPR and First Aid

o TB

o Background Check

o CPI

o Valid Driver’s License

Job Competencies

Required Skills: Must have the ability to demonstrate in excellent interpersonal skills to

effectively work with all levels of personnel and with other agencies. Mature, reliable,

and responsible. Must have clear, concise, and empathetic communication skills. Must

be culturally sensitive and aware. Must know the basics of ethics, HIPPA standards, and

problem solving. Must display sound judgment, a good work ethic, and personal

integrity. Must have knowledge of community resources, outreach, mental/behavioral

health issues in Hawaii. Ability to relate effectively to individuals with a mental illness,

substance abuse issues, co-occurring disorders, and homelessness.

Technical Skills: Basic use of a computer and/or tablet, Microsoft Word Suite, fax

machine, telephone, and text and chat features on the telephone or computer.

Working Environment: May be in modern well-lighted, air-conditioned work areas

and/or in community-based settings; provides outreach in the community.

Indoor/outdoor, open windows or air-conditioned environment; may require some

travel, over-time, weekend, and holiday work, working with other Federal and/or State

accounting bodies, noise level in the environment may be moderate to noisy. Exposure

to infectious diseases is a relatively common risk.

Job Description

Special Demands: Ability to respond effectively to the varied needs and demands of

multidisciplinary team members. Have access to helpful, adequate, competent, and

continuous supports and services. Harnesses the active involvement of team members

in taking ownership of aspects of program development, including operations, record

keeping, staff/client education, treatment planning, and quality assurance activities.

Consumers Served: CARE Hawaii will provide services to any individual who meets the

criteria of the program, eighteen (18) years or older who have a serious mental illness

(SMI). The treatment of individuals should reflect an understanding of the

developmental needs and range of treatment needs for each client.

Physical Demands:

1. Sit for long periods of time, use of proper ergonomics, the ability to focus on a

computer and/or tablet for long periods of time.

2. Ability to assist with the high volume of calls; ability to communicate effectively

both verbally and in writing is essential.

3. Ability to follow instructions and motivation to refer to various rehabilitative

groups. Frequent reading of printed materials is essential.

4. Listening skills necessary to effectively respond to consumers.

5. Ability to identify and support the daily interactions and to deal with each

situation in a stress-free manner.

Duties and Responsibilities:

Essential Functions

  • Directs the day-to-day operations of the team, including scheduling, staff

meetings, Individualized Recovery Planning (IRP) meetings, planning and

coordination of treatment activities.

  • Ensure program services are provided to consumers during the day and evening

office hours between 7:30am and 9:00pm. Service hours shall be provided seven

(7) days a week including holidays and based on consumer needs.

  • Ensure that the consumer to staff ratio is maintained so that all consumers

shall be provided with the intensity of care that is needed.

  • Ensure a comprehensive assessment of psychiatric history, mental status, and

diagnosis; physical/dental health; use of drugs or alcohol;

education/employment; social development and functioning; activities of daily

living; and family structure and relationships upon admission.

  • Ensure documentation of actual time engaging in collateral and telephonic

contacts with community agencies, families and consumers to be charted on a

progress note within twenty-four (24) hours of the interaction.

  • Collateral contact with CBCM co-workers and CBCM psychiatrist, and

pharmacies to be documented in a general log note.

Job Description

  • Ensure/coordinate all crisis services provided twenty-four (24) hours per day

either by phone or face to face contact for crisis response by case managers or

members of the case management (CM) program.

  • In coordination with the Programs Director update clinical policies and

procedures.

  • Assure all CM staff have initial and annual Performance Evaluations on file with a

signed Job Description.

  • Plans, directs, coordinates, and evaluates client admission process and

treatment, rehabilitation, and support services of the program.

  • Individual, side-by-side sessions in which the CM TL accompanies an individual

staff member to meet with consumers in regularly scheduled or crisis meetings to

assess staff performance give feedback and model alternative treatment

approaches.

  • Regular meetings, with individual staff to review and assess staff performance

and give feedback on documentation such as progress notes, assessments, IRP's

and reviews

  • Assign the most appropriate staff to the individual treatment team.
  • Ensure all CM's follow the contract mandates for initial face to face with

consumers:

o CCS: Within three (3) business days of consumer's admission date.

o AMHD: Within twenty-four hours of referral when the consumer is in a short

team crisis stabilization service. Within three (3) business days of referral when

the consumer is in the hospital, or within five (5) business days for referral from

a community agency

  • Ensure CM complete an integrated intake assessment for each consumer

referred to CM Services. The assessment shall be completed through face-toface

contacts with each consumer and significant others involved in each

consumer's treatment and recovery. See contract for integrated assessment

details.

  • Ensure CM provides each consumer with an individualized, coordinated, master

recovery plan, referred to as an Individual Recovery Plan (IRP) that complies with

the requirements of the DIVISION "policy and Procedure #60.648 on Recovery

Planning.

  • Ensure all recovery plans and clinical records submitted by CM without a

bachelor’s degree be co-signed by QMHP.

  • Ensure all Case Managers and Program Managers obtain at a minimum, sixteen

(16) hours of continuing education and training units each year. Continuing

education and training units may be obtained through attendance at D1VIS10N

sponsored trainings, or those sponsored by other professional organizations, and

should include by not limited to fraud and abuse prevention.

Job Description

  • Directs and coordinates, for each client, the comprehensive assessment of

psychiatric history, mental status, and diagnosis; physical and dental health; use

of drugs or alcohol; education and employment; social development and

functioning; ADL.

  • Participate in staff recruitment, interviewing, hiring, work assignments, orientation.
  • Adhere to the program budget, participate in finance reporting and meetings,

as requested.

  • Process progress notes daily and review and approve timesheets and expense

reports, monitoring for accuracy and/or fraud and abuse.

  • Supervises medical records management; carry out and document QA

activities.

  • Ensures contract requirements are fulfilled by all team members.
  • Works with Psychiatrist/APRN Rx and staff to establish appropriate and ongoing

process and outcome indicators and periodic program evaluation studies;

demonstrates improvement where needed.

  • Fosters good relationships among health care providers; maintains dialogue in

interaction with other professionals to identify and resolve existing problems, as

well as to explore new avenues for service; participates/contributes to meetings

pertinent to area of responsibility; promotes quality client care through a spirit of

cooperation with other staff.

  • Participates in strategic planning, identifying, and aligning with growth

opportunities through analyzing trends in client mix and proposes

appropriate changes in operations to accommodate client/staff needs;

evaluates our competitive position and proposes appropriate marketing in our

service area.

  • On-call for team 24/7 for crisis intervention and for staff supervision and

consultation, as needed.

  • Demonstrates self-evaluation skills and takes responsibility for and appropriate

action to remedy or reduce limitation; independently seeks formal and informal

professional development activities to ensure the highest standard of knowledge

and skill.

  • Ensure that authorizations are available for the various services that the client is

receiving by obtaining, tracking, and approving all authorizations from contract.

  • Ensure all acuity levels are met:

o Level 1- One face to face (FTF) time a month

o Level 2- Bi-weekly one FTF one telephonic

o Level 3- Two FTF Two telephonic

o Level 4- One FTF One telephonic weekly

  • Provide clinical oversight for case managers.
  • Ensure that all case managers meet all CCS Metrics:

  • New intake thirty (30) day assessment completed in thirty (30) days. Recovery

plan within sixty (60) days

  • Upon discharge from hospital seven (7) day follow up by licensed provider.

Ensure BH inpatient form is completed.

  • 90% acuity met for the month.
  • Complete the CCS quarterly report within the designated time frame.
  • Ensure all authorizations are accurate and within correct time frames of

consumer's eligible period.

  • Ensure all progress notes are completed within the twenty-four (24) time.
  • Ensure all case assessments and recovery plans are completed every six (6)

months.

  • Prepare and complete all audit requirements every three (3) months.
  • Complete monthly Team reports within time guidelines.
  • Provides required documentation in a timely manner
  • Willingly performs other duties/responsibilities as requested

Additional Duties

1. Assist with Quality Improvement development and monitoring

2. Participate in team meetings

3. Prepare written or electronic reports as requested

4. Interfaces with CARE Hawaii, Inc. staff

5. Participates in conflict resolution as necessary

6. Maintain chart filing, ensuring charts are properly completed thoroughly and

monitor charts to ensure charts are always compliant and audit ready
