Care Options Support Services is a Jacksonville, FL based company that specializes in providing services to individuals with mental and physical disabilities. We are currently seeking (PRN) Direct Support Professionals to join our team in Jacksonville, FL.

Job Description:

The Direct Support Professional is responsible for assisting clients with daily living activities and providing support for the client's transition into the adult world. DSP’s will work closely with the behavioral health team to develop and implement services that meet the client’s need for independence, self-management, and social engagement.

Duties and Responsibilities:

• Provide direct support to clients as they complete daily activities of living.
• Provide direct support to clients as they complete non-medical care related tasks, such as medication administration, bathing, dressing, etc., as directed by the client's health care provider.
• Provide direct support to clients as they engage in leisure activities of living such as playing with toys, sandbox sand, and other outdoor activities.
• Provide direct support to clients as they perform light housekeeping tasks such as laundry, feeding self, etc., as directed by the client's health care provider.
• Provide direct support to clients as they interact with members of the home health or skilled nursing facility staff and/or family members through social interaction.
• Provide direct support to clients as they perform light household maintenance tasks such as mowing, cleaning, eating, dressing, etc., as directed by the client's health care provider.
• Provide direct support to clients as they perform non-medical care related tasks such as cooking, meal preparation, housekeeping, medication administration administration, bathing, etc., as directed by the client's health care provider.
• Complete documentation of assigned services accurately and in a timely manner.
• Willingness to learn new skills and techniques to provide assistance in developing life-long services for our clients.
• Other duties may be assigned.

Requirements:
Must be 18 years of age or older. Must have a high school diploma or GED certificate with a valid driver’s license and a good driving record (if applicable). Must be willing to submit to a level background check. Must possess or be able to obtain CPR First Aid certification within 30 days of employment (or request)

Job Type: Full-time

Pay: $15.00 - $16.00 per hour

Schedule:

• 8 hour shift (more or less) as needed

Ability to commute/relocate:

• Jacksonville, FL: Reliably commute or planning to relocate before starting work (Required)

Experience:

• Caregiving: 1 year (Required)

Shift availability:

• Afternoon to Evening Shift (Preferred)
• Overnight Shift (Preferred)

Work Location: In person

Job Type: Part-time

Pay: $15.00 - $16.00 per hour

Expected hours: 8 – 24 per week

• 12 hour shift
• 4 hour shift
• 8 hour shift
• Evening shift
• Night shift
• On call
• Overnight shift
