**Job Responsibilities**
Provide patients with personal care directed by the RN and plan of care.
Assist patients in bathing, care of mouth, skin, and hair, and assist patients in and out of bed and with ambulation.
Perform turns of patients in bed and assist in positioning patients for comfort as well as changing bed linens as needed.
Report changes in patient's condition and needs.
Prioritize patient comfort, quality of life, and safety.
Maintain up-to-date patient records and chart in a timely manner according to policy.
**Required Skills and Qualifications**
Education: High School Diploma or GED preferred.
Experience: 1 year experience in patient care in a home health, long term care, hospice, and/or acute care setting.
Must be able to read, write, verbally report important information about patients.
Current BLS certification.
Possess valid state driver's license and reliable automobile and be willing to operate personal car necessitated by nature of job.
Proof of current automobile insurance with minimum state financial responsibility limits.
Current health certificate/physical examination and TB testing.
Ability to lift up to 50 lbs.
Ability to operate a desktop and laptop computer.
Ability to stand and/or sit for long periods of time.
Ability to climb stairs and to provide services in a patient's home environment.
Ability to lift, pull, push, and manipulate equipment and patients which requires strength, gross motor, and fine motor coordination.
