The Families First Counselor at CarePlus New Jersey will be responsible for a range of critical tasks, including:
In-Home Services: Providing in-home support services and interventions to clients, including case management, linkage to community resources, and inter-agency coordination of treatment.
Assessments and Evaluations: Conducting thorough assessments and evaluations of clients at intake, during ongoing service delivery, and termination.
Care Planning: Formulating relevant goals with measurable objectives that serve as indices for evaluating intervention progress and realistic time frames for achievement.
Documentation: Preparing written documentation of assessments, intervention plans, progress notes, and discharge summaries in the clients' files.
Collaboration: Collaborating with an interdisciplinary team to develop and implement aftercare plans for clients completing program services or accessing other services in the continuum.
