Job Summary:
We are seeking a highly skilled Families First Counselor to join our team at CarePlus New Jersey. As a key member of our mental health services department, you will be responsible for providing in-home support services and interventions to clients, including case management, linkage to community resources, and inter-agency coordination of treatment.
Your primary focus will be on conducting thorough assessments and evaluations of clients, formulating relevant goals with measurable objectives, and documenting all interactions and progress in the client's file. Additionally, you will collaborate with an interdisciplinary team to develop and implement aftercare plans for clients completing program services or accessing other services in the continuum.
Key responsibilities include:
- Providing in-home services and supportive interventions to clients
- Conducting assessments and evaluations of clients at intake, during ongoing service delivery, and termination
- Formulating relevant goals with measurable objectives that serve as indices for evaluating intervention progress and realistic time frames for achievement
- Preparing written documentation of assessments, intervention plans, progress notes, and discharge summaries in the clients' files
