We are seeking a skilled PRN Evaluating Physical Therapist to join our team in Pulaski County, Indiana. As a Physical Therapist, you will be responsible for evaluating and treating patients with physical impairments or disabilities. You will work closely with patients to develop individualized treatment plans and help them achieve their goals.
The ideal candidate will have experience working in a Skilled Nursing Facility and be able to work independently as well as part of a team. You must have excellent communication and interpersonal skills, as well as a strong attention to detail.
Responsibilities:
- Evaluate patients and develop individualized treatment plans
- Provide hands-on therapy to patients or oversee Therapist Assistants
- Monitor patient progress and adjust treatment plans as needed
- Communicate with patients, families, and other healthcare professionals
- Maintain accurate patient records
Requirements:
- Valid Physical Therapist license in the state of Indiana
- Experience working in a Skilled Nursing Facility
- Excellent communication and interpersonal skills
- Strong attention to detail
- Ability to work independently and as part of a team
