MDS Coordinator Job Description
We are seeking a highly skilled MDS Coordinator with experience in long term care to join our team. The MDS Coordinator will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments for our residents in accordance with all applicable laws, regulations, and standards of practice.

Responsibilities:
- Coordinate and complete MDS assessments for all residents in a timely and accurate manner
- Ensure that all assessments are completed in accordance with all applicable laws, regulations, and standards of practice
- Collaborate with interdisciplinary team members to develop and implement care plans for residents
- Ensure that care plans are updated and revised as necessary
- Provide education and training to staff on MDS assessments and care planning
- Participate in quality improvement initiatives and activities
- Ensure that all documentation is complete, accurate, and timely
Requirements:
- Current nursing license
- Minimum of 2 years of experience in long term care
- Experience with MDS assessments
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong attention to detail
- Ability to prioritize and manage multiple tasks
