Job Summary:
The Care Coordinator will be responsible for managing employee schedules, coordinating care for clients, onboarding new clients, and ensuring smooth communication between case managers, clients, and the care team. This position requires a highly organized, detail-oriented individual with strong communication skills and a passion for providing high-quality home health care.
Key Responsibilities:
- Employee Scheduling: Efficiently schedule and coordinate home care staff to ensure timely and effective care for clients.
- Client Onboarding: Sign up new clients by assessing their care needs, explaining available services, and completing necessary intake paperwork.
- Care Management: Oversee and monitor the care of existing clients, ensuring that care plans are followed and any changes in care needs are addressed.
- Collaboration with Case Managers: Maintain regular communication with case managers to ensure continuity of care and provide updates on client progress.
- Documentation: Ensure accurate and up-to-date documentation of client care plans, visit schedules, and all other necessary paperwork.
- Client Support: Serve as the main point of contact for clients and their families, providing assistance and addressing any questions or concerns related to care.
- Problem-Solving: Resolve issues or concerns that arise related to care delivery, scheduling, or client needs.
- Compliance: Ensure all services are provided in compliance with state and federal regulations as well as company policies.
- Community Outreach: Actively engage in outreach to identify and sign up new clients in the community, promoting Care United Home Care Agency’s services.
Qualifications:
Education: Bachelor’s degree required, preferably in Social Work or a related field.
Experience: Proven experience in a care coordination, scheduling, or case management role, preferably in a home care or healthcare setting.
Skills:
- Strong organizational and time-management skills.
- Excellent verbal and written communication skills.
- Ability to work independently and collaboratively with the team.
- Proficiency with Microsoft Office Suite and scheduling or care management software.
- Knowledge of home care regulations and best practices is preferred.

Physical Requirements:
- Ability to sit for extended periods and work on a computer.
