An ideal candidate must be an LPN who is knowledgeable/experienced in dealing with clinical review of home health referrals per Medicare requirements and Home Health regulations. This individual must be detail-oriented, self-motivated, and a team player. Job Description: Clinical Intake Coordinator:
- Acts as a liaison between the referral sources, patient, and internal staff by processing incoming referrals for patients needing home health services.
- Verifies a patient’s insurance benefits and competently explains those benefits to all parties involved with accuracy.
- Clinically reviews the referral documentation to ensure we have all the documents needed to safely initiate care for a patient.
- Works in conjunction with the Clinical, marketing, and other team members to ensure timely initiation of care and to adhere to our standards and applicable laws and regulations.
- Assists with Welcome Calls to new patients to provide a warm and friendly first impression.
- Gathers and verifies information to accurately enter the patient’s information into our EMR, including demographics, physician and insurance information, etc.
- Actively participates in a high-performance environment to not only drive change but truly enhance the quality of patient service.
Qualifications
- Must be a licensed LPN in the state of VA and have 2 years of experience
- Previous home health experience preferred
- Some experience completing insurance verifications and interpreting home health benefits
- Excellent written and verbal communication, time management, and organization skills
- Ability to work in a fast-paced environment
- Goal and results driven
