- Conduct initial and periodic in-home client assessments by meeting prospective clients and families to gather pertinent information relative to their care goals.
- Assist in planning and conducting caregiver orientations.
- Determine & establish care plans, based on client needs and preferences, that represent the best level of non-medical care as needed to keep the client comfortable and safe at home.
- Perform supervisory visits and / or reassessments with clients every 90 days and at least annually with the caregiver present.
- Provide training and in-services to caregiver staff.
- Reporting incidents (as needed) to the Health Care Personnel Registry.
- Perform Competency Verification Checks on care giving staff during on-boarding and annually.
- Maintain compliance with the state regulations affecting home care in all areas listed aboveQualifications :
- Current and valid North Carolina state RN license
- Non medical home care experience preferred
- Excellent communication, interpersonal and assessment skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic approach to the elderly and disabled persons desiring non-medical in-home patient care.
- Reliable transportation and willingness to travel to client homes as needed.
