Care Connect is seeking qualified home health aides and personal care assistants to join our team in Queens. As a home health aide, you will play a vital role in providing care to our clients, ensuring their safety and well-being at home.

Key responsibilities include:

• Providing personal care to patients, including bathing, dressing, and grooming
• Assisting with mobility and transfers to ensure patient safety
• Administering medications as directed by healthcare professionals
• Monitoring vital signs and reporting changes or concerns to medical personnel
• Offering emotional support and companionship to patients and their families
• Maintaining a clean and safe environment for patients

Requirements:

• Certified Personal Care Assistant (PCA) or Certified Home Health Aide (HHA)
• Understanding of HIPAA regulations and patient confidentiality
• Ability to cook simple meals and perform light housekeeping duties as needed
• Satisfactory completion of New York State Health Department-approved HHA Training Program or equivalency
• Demonstrated adequate literacy and simple arithmetic skills
• Age 18 or older
