At CareConnect, we're seeking compassionate and skilled Licensed Practical Nurses (LPNs) to join our team of dedicated professionals. As an LPN with us, you'll play a vital role in providing exceptional care to patients in the comfort of their own homes.
Duties:
- Provide direct patient care in accordance with the state Nurse Practice Act.
- Implement the plan of care initiated by the registered nurse.
- Ensure accurate and timely documentation that aligns with the patient's plan of care.
- Assess and educate patients and their families/caregivers on self-care techniques and relevant information related to the patient's diagnosis and care plan.
- Coordinate home health services by communicating identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
- Use and prepare medical equipment and supplies effectively, ensuring adherence to aseptic techniques.
- Participate in personal and professional growth and development activities to enhance clinical skills and knowledge.
- Perform additional duties as assigned by the registered nurse to support patient care and team collaboration.
Requirements:
- A graduate of an accredited practical nurse or vocational nursing program.
- A current, active license as an LPN in the state of Massachusetts (MA).
- An up-to-date CPR/BLS certification.
- A minimum of three years of nursing experience, preferably in community/home health or medical/surgical settings.
- The ability to communicate effectively in English, both spoken and written.
- A valid state driver's license and reliable automobile.
