Overview
CareConnect is seeking compassionate Licensed Practical Nurses to provide exceptional care to patients in their homes. As a vital member of our team, you will be responsible for delivering high-quality patient care, adhering to the state Nurse Practice Act.
Duties and Responsibilities
- Implement the plan of care initiated by the registered nurse.
- Assess and educate patients and their families/caregivers on self-care techniques and relevant information related to the patient's diagnosis and care plan.
- Coordinate home health services by communicating identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
- Use and prepare medical equipment and supplies effectively, ensuring adherence to aseptic techniques.
- Perform additional duties as assigned by the registered nurse to support patient care and team collaboration.
Required Skills and Qualifications
- Graduate of an accredited practical nurse or vocational nursing program.
- Current, active license as an LPN in the state of Massachusetts (MA).
- Up-to-date CPR/BLS certification.
- A minimum of three years of nursing experience. Experience in community/home health or medical/surgical settings is preferred.
- Valid state driver's license and reliable automobile.
- Authorization to work in the U.S. and proof of eligibility if necessary.
- Fluency in English, both spoken and written, to communicate effectively with patients and the healthcare team.
- Strong understanding of HIPAA regulations and commitment to maintaining patient confidentiality at all times.
- Current health certificate/physical examination and TB testing results.
