Job Overview: We're currently seeking a skilled Registered Nurse to join our team in Worcester County, MA. The ideal candidate will possess excellent clinical skills, a strong understanding of HIPAA regulations, and the ability to effectively communicate with patients and their families.
Duties and Responsibilities:
- Perform initial and ongoing patient assessments based on individual needs and physician orders.
- Develop and update written plans of care, ensuring timely re-evaluation as necessary.
- Provide skilled nursing care within the scope of practice, adhering to federal, state, and Agency regulations.
- Document care and services in compliance with relevant laws and guidelines.
- Lead case conferences to discuss multidisciplinary team responsibilities, patient progress, and future care plans.
- SUPPORT PATIENTS AND FAMILIES BY PROVIDING TAILORED EDUCATION BASED ON THEIR UNIQUE NEEDS AND PREFERENCES.
Qualifications: To be successful in this role, candidates must meet the following requirements:
- Be a graduate of an accredited school of nursing.
- Hold a current, active Registered Nurse license in the state of Massachusetts.
- Have up-to-date CPR/BLS certification.
- Provide an updated resume outlining relevant experience and qualifications.
- Hold a valid state driver's license and have access to a reliable vehicle.
- Be authorized to work in the U.S. and provide proof of eligibility if necessary.
- Be fluent in English, both spoken and written, to ensure effective communication with patients and the healthcare team.
- Have a strong understanding of HIPAA regulations and demonstrate a commitment to maintaining patient confidentiality at all times.
- possess one year of acute care, home healthcare, community health, or outpatient facility experience within the last two years.
- Provide current health certificate/physical examination and TB testing results.
- Be at least 18 years of age.
