Duties:
- Deliver direct patient care in accordance with state regulations and standards.
- Implement care plans initiated by registered nurses, ensuring seamless coordination of care.
- Document patient progress accurately and timely, adhering to industry standards.
- Educate patients and their families/caregivers on self-care techniques and relevant information related to diagnosis and care plan.
- Coordinate home health services by communicating identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
- Use and prepare medical equipment and supplies effectively, maintaining adherence to aseptic techniques.
- Participate in personal and professional growth and development activities to enhance clinical skills and knowledge.
Requirements:
- Graduate of an accredited practical nurse or vocational nursing program.
- Active license as an LPN in the state of Massachusetts (MA).
- Current CPR/BLS certification.
- Minimum three years of nursing experience; community/home health or medical/surgical settings preferred.
- Valid state driver's license and reliable automobile.
- Able to work in the U.S. and provide proof of eligibility if necessary.
- Fluent in English, both spoken and written, to communicate effectively with patients and healthcare team.
- Strong understanding of HIPAA regulations and commitment to maintaining patient confidentiality.
