About the Role
As a Patient Advocacy Associate with CareConnect, you will be responsible for providing personalized care services to patients in their homes, focusing on their physical, emotional, and social needs.
- Deliver high-quality patient care, addressing their medical, nutritional, and personal needs.
- Work collaboratively with healthcare professionals to develop and implement individualized care plans.
- Maintain accurate records of patient care, including progress notes and medication administration.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
To excel in this role, you must possess:
- A certification as a PCA or HHA.
- An understanding of HIPAA regulations and patient confidentiality.
- Ability to engage patients in activities that promote mental and physical stimulation.
- Demonstrated experience working with patients with diverse needs and backgrounds.
- Excellent communication and interpersonal skills.
- Be at least 18 years old.
