About CareConnect
We are a national leader in home care, providing exceptional care that our clients rely on to stay safe at home. As a licensed practical nurse with us, you will need to be a licensed practical nurse in the state of MA, and provide direct patient care in accordance with the state Nurse Practice Act.
Responsibilities:
- Patient Care: Provide direct patient care in accordance with the state Nurse Practice Act, including assessing and educating patients and their families/caregivers on self-care techniques and relevant information related to the patient's diagnosis and care plan.
- Team Collaboration: Collaborate with other healthcare professionals to ensure accurate and timely documentation that aligns with the patient's plan of care, and communicate identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
- Equipment and Supplies: Use and prepare medical equipment and supplies effectively, ensuring adherence to aseptic techniques.
- Professional Development: Participate in personal and professional growth and development activities to enhance clinical skills and knowledge, and perform additional duties as assigned by the registered nurse to support patient care and team collaboration.
Requirements:
- Must be a graduate of an accredited practical nurse or vocational nursing program.
- Must hold a current, active license as an LPN in the state of Massachusetts (MA).
- Must have an up-to-date CPR/BLS certification.
- Has to have a minimum of three years of nursing experience. Experience in community/home health or medical/surgical settings is preferred.
- Must provide an updated resume outlining relevant experience and qualifications.
- Must have a valid state driver's license and reliable automobile.
- Must be authorized to work in the U.S. and provide proof of eligibility if necessary.
- Must be fluent in English, both spoken and written, to communicate effectively with patients and the healthcare team.
- Must have a strong understanding of HIPAA regulations and demonstrate a commitment to maintaining patient confidentiality at all times.
- Must provide current health certificate/physical examination and TB testing results.
- Must be at least 18 years of age.
